For your convenience, we accept American Express, Visa, MasterCard, Discover, and third-party financing through CareCredit. We are committed to providing high-quality care at a fair cost, and payment is due at the time services are rendered unless arrangements have been made in advance. If you have any questions regarding your account, our team is here to help. Many concerns can be resolved quickly with a simple phone call.
If you have dental insurance, we are happy to assist you in receiving the maximum benefits allowed by your plan. When scheduling your appointment, please provide your insurance information so we can verify your benefits and offer an estimated copay before your visit. Because every plan is different, we can only offer an estimate, and final coverage is determined by your insurance company once your claim is processed.
Please remember that submitting an insurance claim does not change your responsibility for any remaining balance. Most insurance companies send payment to our office within four to six weeks. Any remaining balance after insurance has paid is your responsibility. We are not responsible for coverage denials, and all copays and deductibles are due at the time of service.
You are fully responsible for all fees charged by our office regardless of your insurance coverage. If you ever have a question about your statement or balance, please contact us at (480) 400-4112 so we can assist you.
