Getting Ready for Your Visit
Getting Ready For Your First Visit
Your first appointment will usually include a consultation that explains your diagnosis and the treatment options available to you. In some situations, treatment may be completed during the same visit. If your medical history or treatment needs are more complex, an evaluation may be required first and the procedure scheduled for a separate appointment.
Please help us prepare for your visit by bringing the following items
- Your referral slip and any recent X-rays if you have them.
- Completed Patient Registration Paperwork
- A list of medications you are currently taking.
- Your medical or dental insurance information so we can assist with claim processing.
Important Notice:
Patients under the age of 18 must be accompanied by a parent or guardian at the consultation.
Please inform our office in advance if you have a medical condition that may require special attention before treatment. Examples include high blood pressure, diabetes, artificial joints or heart valves, rheumatic fever, or if you take medications such as blood thinners or heart medications. If your physician or dentist recommends antibiotics before dental procedures, please notify us.
X-Rays:
If your dentist has taken X-rays within the last six months, you may request that they send them to our office. Please be aware that pre-operative and post-operative X-rays taken in our office are required for proper documentation and care.
